SUMMARY

Led the transformation of vaccine distribution and inventory management systems. Over a five-week period, the implementation of  a robust tracking and monitoring process for approximately 300,000 vaccines, from shipping to administration. Utilizing Salesforce.com, eClinicalWorks, and the Microsoft suite, the project team developed a real-time tracking system that integrated data across three platforms. This system ensured the accuracy of patient records, tracked vaccine lot numbers, and managed inventory based on expiry dates and availability. Key innovations included the development of a QR code system for real-time vaccine tracking and a scalable process adopted by the CDC for use by other organizations. Despite the challenges of limited resources and disconnected platforms, we achieved a 12-hour turnaround for inventory updates and established a sustainable system still in use today.This project not only streamlined vaccine distribution during the pandemic but also enhanced the quality assurance processes for future public health initiatives.

SUMMARY

Project Summary: A 12-week initiative to enhance the CMS Quality performance ratings for the Pittsburgh market’s Hypertension (HTN) Clinical Program. Utilizing monday.com, I developed a qualitative data collection tool to survey 18 primary care offices, aiming to elevate the program’s performance from a 4.5 to a 5. Key challenges included inconsistent processes for taking and documenting blood pressure across offices. Through cross-team communication and coordination with the quality and project management teams, we implemented a standardized process for blood pressure measurements. This involved continuous feedback collection, EMR updates for efficiency, historical data tracking, and ongoing staff education.The project resulted in consistent blood pressure measurement practices, identification, and correction of data errors, and improved data entry processes through specialized EMR training. These improvements led to enhanced annual closeout practices, increased cost efficiency, and better budget allocation for equipment and training. The success of this initiative prompted Enterprise Quality to request a tutorial presentation for cross-market integration of the quality improvement process, showcasing the potential for broader implementation and further process enhancements.

SUMMARY

The development and design of a comprehensive health and wellness department for a community-based nonprofit in Central Texas. Over a 6-month period, our team focused on addressing critical community health needs through a holistic approach, targeting social determinants of health, and enhancing the quality and accessibility of programming. Key initiatives included implementing sustainable practices, collaborating with environmental health and youth development sectors, and integrating educational empowerment strategies to build community capacity. Our efforts resulted in securing $2 million in grant funding, which was pivotal in supporting community needs during the COVID-19 response and developing tools for tracking funds and addressing health disparities. The program's success was evident through improved health indicators in the community, effective interventions addressing social determinants of health, and increased community engagement, particularly in disproportionately disadvantaged areas. The lessons learned and best practices identified provide valuable insights for future implementations and replication in similar contexts. This project not only established a robust health and wellness department but also demonstrated the profound impact of targeted community health initiatives and the importance of addressing broader social determinants for sustainable community development.
